CHOP
The Community Healthcare Outreach Program (CHOP) falls under Nirvana’s Population Health Office. By identifying specific patient segments, CHOP delivers coordinated, APN-led care to those with multiple chronic conditions. Our mission is to reduce the burden of chronic diseases and improve health outcomes through early intervention and education. CHOP ensures that underrepresented and underserved communities have access to affordable, quality care—especially as healthcare challenges evolve.
SHOP
The Senior Health Outreach Program (SHOP) focuses on adults aged 65 and older. Through community-based services, SHOP addresses chronic disease management, fall prevention, and cognitive health. Our dedicated team collaborates with seniors, caregivers, and community partners to support independence, improve health literacy, and deliver care tailored to the needs of older adults. SHOP bridges the gap in access and empowers seniors to live healthier, more fulfilling lives.
