Mobile Phone Users
Step 1: Go to Apple App or Google play store
Step 2: Download the IMS Patient App or click on one of the buttons below
Step 3: Enter our clinic number P1627670
Laptop / Desktop Users
Step 1: Select the button below to register for our patient portal
Step 2: On the RIGHT side of your screen click on the Register here (under the log-in button)
Step 3: Create a username and Password
NOTE: Our Patient Portal has a Spanish option. CLICK the globe in the upper right corner and select Spanish.
